Optimizing Care Delivery Impact Through The Team-Based Care Approach
During this live, all-day training course the learner will gain knowledge and skills to work collaboratively in a multidisciplinary setting to assure that care is coordinated, safe, and patient-centered. Participants are required to complete this course with a posttest score of 80% or greater in order to bill for Priority Health and BCBSM PDCM (Provider Delivered Care Management) codes. This requirement must be met prior to billing services for Priority Health and within six months of starting to bill for BCBSM PDCM.
At the conclusion of this training course, the participant will be able to:
- Effectively empower patients and their families.
- Engage patients in self-management and health behavior change.
- Positively affect patient self-care practices and health outcomes.
- Provide comprehensive assessment including a holistic evaluation social determinants of health and care planning using shared decision making.
- Implement evidence based interventions and advocate for the right care, at the right time and in the right place.
This training is fully vetted with care management leaders and training attendees. Curricula development, facilitated by MICMT, was co-developed with trainers across Michigan, to include IHP, Lynn Klima, PTI, and NPO. Mi-CCSI ensures trainers are well versed in the concepts and have extensive experience to offer insight and application of the skills, tools, and information covered.
*** Mi-CCSI is an approved vendor for MICMT. This training provides 7 credit hours of BCBSM PDCM required education. Blue Cross Blue Shield Of Michigan has a PDCM training fee reimbursement available for up to $500 within capped agreement with BCBSM. See your physician organization’s BCBSM PDCM agreement for more details.

This is an all day live virtual training event. 7:45 am to 4:30 pm
Audience
This course is intended for nurses, social workers, advanced practice providers, medical assistants and community health workers who are new to care management or care coordination, or seeking to expand their knowledge and skills as part of a Patient Centered Medical Home, Accountable Care Organization, or other transitional care role.
Faculty
Robin Schreur, BS, RN, CCMRobin Schreur is a trainer for the Michigan Center for Clinical Systems Improvement (Mi-CCSI). She served as the Clinical Lead Care Manager in Ambulatory Care at Spectrum Health from 2012 until 2020. During this time, she was part of the collaborative care model implementation team. Her aim is to instill the importance of patient-centered care across the continuum of care. Robin has worked in a variety of settings including insurance-based and primary care practices, and as a behavioral health nurse at an inpatient facility. She has extensive experience in training and onboarding hundreds of clinicians in care management roles and motivational interviewing.
Sue Vos, BSN, RN, CCMSusan Vos is the Program Director at the Michigan Center for Clinical Systems Improvement (Mi-CCSI). She has experience in working with multi-disciplinary teams to provide care for complex patients, extensive knowledge and expertise in practice transformation, developing and implementing care management curricula based on evidence-based guidelines, training of care manager and other clinical practice staff, implementing care models, and driving quality improvement strategies. She is the current president of the Case Management Society of America: Grand Rapids/Kalamazoo Chapter.
Lynn Klima, DNP, APRN, CNE, ANP-BCDr. Klima is an adult nurse practitioner with over 40 years of experience across acute, primary care, and community-based settings. She obtained her Doctorate in Educational Leadership in 2018 from American Sentinel University and is a Certified Nurse Practitioner through the American Nurse Credentialing Center and a Certified Nurse Educator through the National League of Nursing. Across the last 14 years, she has been an academic associate nursing professor with a teaching focus on nursing leadership, therapeutic communication, community health, geriatrics, and hospice/ palliative care at the graduate and undergraduate levels.
Her work history includes executive leadership responsibilities that focused on leadership development, performance improvement, care management delivery models, and care coordination initiatives to support the complex chronically ill.
Dr. Klima is dedicated to engaging staff in care delivery improvements. Her doctoral research focused on understanding how to support nurses in engaging patients in providing self-management support to their patient populations. Her primary interests as an implementation scientist include integrating evidence-based practices that focus on patient safety, transition management, patient-centered care, and leadership development across care settings.

Providers
The AAFP has reviewed Team Based Care, and deemed it acceptable for AAFP credit. Term of approval is from 12/22/2023 to 12/20/2024. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Credits Available: Up to 6.75
Full continuing education credits are achieved by the successful participation and completion of the entire live training course and all required pre-/post-tests and evaluations.
Training continuing education certificates will be issued by Mi-CCSI after the attendee completes the associated evaluation.
PDCM Training completion certificates will be issued by MICMT after the attendee completes the associated post-test and evaluation.
