This two-part video series is intended for clinical care team members to provide an awareness of the impact of social needs to effectively manage patient health.

Video 1: Community Health

This Social Determinants of Health (SDOH) video is provided by the Michigan Center for Clinical Systems Improvement. This video walks the viewer through defining patients social needs. This video is provided as a resource to bring awareness to clinical teams on the impact of social needs to a patient's overall health.

Video 2: Kathy's Story

This video walks the viewer through a patient's story outlining her social and economic struggles with meeting her health goals.
