Serious Illness Conversations: Late Stage Chronic Kidney Disease
This learning series is comprised of two independent vignettes that explore person-centered approaches to decision making for patients with late-stage kidney disease emphasizing the importance of preparedness planning, understanding patient values, and considering hospice care options. Additionally, the learner will explore person-centered approaches for pain management for patients with late-stage kidney disease. These vignettes are organized as independent learning activities that can be completed separately or as the whole series. The learning series is an addendum to the all-day Serious Illness training. To expand the learnings on managing populations with a serious illness, it is highly recommended to take the all-day Serious Illness training.
At the completion of this learning series, the learner will be able to:
- Review key considerations to include in the patient-provider serious illness discussion when a patient has Late-stage Kidney Disease
- Describe common painful conditions encountered in patients with chronic kidney disease
- Discuss special considerations for pain management medication selection when managing Late-stage Kidney Disease

Session #1: Serious Illness Conversation: Patient with Late-Stage Chronic Kidney Disease
In this vignette, Drs. Stacie Levine and Aziz Ansari discuss person-centered approaches to decision making for patients with late-stage kidney disease, emphasizing the importance of preparedness planning, understanding patient values, and considering hospice care options. They highlight key themes from a national survey of nephrologists and provide practical advice for primary care clinicians on how to engage in serious illness conversations and manage dialysis choices with their patients.

Session #2: Serious Illness Conversation: Pain Management with Late-Stage Kidney Disease (LSKD)
In this online learning activity, Drs. Stacie Levine and Aziz Ansari discuss the person-centered approaches and considerations for pain management for the population with LSKD. During this recorded session, the physicians review managing medications to optimize comfort and minimize risks.

The course is intended for providers, nurses, social workers, advanced practice providers, and other care team members who are seeking to expand their knowledge and skills.
Faculty
Aziz Ansari, DO, SFHM, FAAHPM, FACPDr. Aziz Ansari is a Professor of Medicine, and Associate Chief Medical Officer, Clinical Optimization and Revenue Integrity at Loyola University Medical Center in Maywood, Illinois. He is a practicing and board-certified
hospitalist and palliative care physician. One of his responsibilities includes ensuring that palliative care is being utilized effectively in the medical center. He is passionate on the need for developing strong primary palliative care skills across all disciplines. Dr. Ansari speaks on topics such as communication, fighting implicit biases through cultural humility, and the role of palliative care in improving health care delivery.
Dr. Stacie Levine is Professor and Chief of Geriatrics and Palliative Medicine at University of Chicago and a national leader in geriatrics and palliative medicine education. She Co-PI of a large-scale, multi-centered project that has been consecutively funded by the Coleman Foundation since 2010 and involves longitudinal training in palliative medicine clinical skills, program development, leadership engagement, and patient advocacy for physicians, advanced practice practitioners, psychologists, and social workers. The Coleman Palliative Medicine Training Program boasts hundreds of graduates and expands to a national and international audience.
